Purpose of Form

Use this form to request a review by the Fraternity & Sorority Life Review Board.

It is designed for concerns about chapter culture, questionable behavior, or situations that may benefit from educational redirection. Include as much detail as possible — dates, individuals or chapters involved, and any supporting documentation.

Important: Alleged violations of university policies, state or federal law, or serious conduct issues will be forwarded to the Dean of Students Conduct Office for investigation. The Review Board focuses on fostering accountability, values-based decision-making, and positive growth within our fraternity and sorority community.
